


INTEGRATED BEHAVIORAL
HEALTH ACTION COALITION
The Integrated Behavioral Health Action Coalition (IBHAC) is made up of primary care, mental health, and substance use providers, as well as local hospitals, county agencies, health plans, mental health advocacy organizations, and individual community members. Together, IBHAC members plan to improve system-level integration of primary care and behavioral health services for all persons and to create a collaborative, integrative, and inclusive system of whole-person care in Santa Cruz County.

HIP also convenes a monthly IBHAC Workforce Sub-Meeting to discuss behavioral health and health social workers workforce. The meetings are facilitated by Jasmine Nájera, LCSW, Chief Executive Officer at the Pajaro Valley Prevention and Student Assistance (PVPSA).

Best Practices in Behavioral Health Integration

This event showcased five behavioral health focused programs in four regional counties all with the goal of achieving whole-person care. Representatives from Merced County focused on their Whole Health Partnership's Data Dashboard Project and the HOPE Medical Respite Program. Monterey County's Health Department shared their Whole Person Care Pilot Project. Members of Santa Cruz County presented about their Integrated Behavioral Health Hospital Transitions Pilot and Central Coast Recovery Options. San Mateo County described their Integrated Medication Assisted Treatment Program.
